Please Note: We charge for any appointments cancelled or no-show within 24 hours. We pride ourselves on the efficiency of our office staff to have you see the Doctor or Nurse Practitioner in a timely manner.
Patients are encouraged to download and fill out any of the applicable forms prior to their visit. Having these filled out in advance will decrease waiting time in the office.
New patients will want to download all of these forms. Please review carefully, and if you have questions we will be happy to answer them during your visit.
If there are medical records from other doctors that you would like to have transferred to one of our providers at MPMA, please fill out a Medical Records Release form and bring it with you to your visit.
Before your first visit
Call your insurance company to verify that your chosen provider (our doctor) accepts your insurance and is in your network.
Download, print and fill out the new patient forms and bring them with you to your first appointment.
Your insurance verification will go more smoothly if we have a copy of your insurance card prior to your first visit. You may email* a copy (front and back) of your insurance card to email@example.com prior to your first appointment.

Current list of medications (or bring your medications with you)
Preferred pharmacy name, address and phone number(s)
If you do not bring your insurance card to your visit and we cannot verify your insurance, you will be considered a ‘self-pay’ patient until your insurance can be verified.
We will charge your credit card at the time of visit. The charge can be reversed if we receive, and can verify, your insurance information within two (2) business days. We appreciate your understanding in this matter.
